heart rhythm

What causes palpitations, and when should they be investigated?

Palpitations are common, but their pattern and associated symptoms help determine whether further assessment is appropriate.

What are palpitations?

Palpitations are an awareness of the heartbeat. People may describe a racing, pounding, fluttering or skipped sensation. They can be brief or sustained and may occur at rest or during activity.

Why the pattern matters

Useful details include when the sensation starts, how long it lasts, whether it is regular and whether it is associated with dizziness, breathlessness, chest discomfort or fainting. Caffeine, alcohol, stress, medicines and underlying rhythm conditions are among the factors a clinician may consider.

How palpitations are assessed

An ECG records the rhythm at one point in time. When symptoms are intermittent, longer monitoring may sometimes be considered. The appropriate approach depends on the history and clinical findings.
